Actions for improvement in the dissemination and implementation of the acute pain assessment protocol

Pain assessment is one of the daily activities that nurses perform systematically throughout their shifts; nevertheless, as pain is a subjective sensation, its quantification is not routinely recorded but rather through nursing comments. With the implementation of the SERGAS acute pain protocol at our Hospital Complex and the use of pain assessment scales, we aimed to evaluate whether the use of nursing diagnoses of Pain and Risk of Pain and their associated activities increases, thus contributing to the quality of care and documentation, in addition to understanding pain assessment as the fifth vital sign.
